Instructions for Completing AML Contractor Form

 

Part A: Self-explanatory.

Part B: Self-explanatory.

Part C: Certifying and updating information in the AVS.

There are 3 options for the AML contractor to select from. Before completing Part C, you should first contact our office at 800.643.9748 and request a copy of your company's OFT (organizational family tree) from the AVS. Once you have obtained a copy of your company's information as contained in the AVS, select the appropriate option that applies to your situation.

Note: If you are certifying that information is accurate or that information needs to be modified in some manner, you must always attach the printout you received from the AVS Office.

If you have determined that your company has not yet been established in the AVS, select that option in Part C, sign the form, and provide the requested information in Part D of the form.

Part D: Completion of Part D is only required if your company does not exist in the AVS or if existing information is out of date and needs to be modified.

 

Once you have completed and signed the form, please provide it, along with any printouts you received from the Applicant/Violator System, to the AML Program Specialist you are working with.
